I totally did this once. Bought the car 3 weeks earlier, I was used to my old car. Truck on the highway starts coming into my lane so I try to accelerate. Went from the top of 3rd straight to 2nd and bye bye engine.

Ouch. I have missed a shift from time to time but never wrecked an engine doing it. But my car has a diesel so maybe a little more forgiving.
