I understand your general concerns but thankfully for this community the onlyfans example doesn't really apply.
I also would have preferred a solution where it's up to the individual communities to disable or enable downvotes.

I've got the image from https://realbooru.com/index.php?page=post&s=view&id=780239 and it is tagged with shiri allwood. According to a reverse image search that seems to be correct.
I think it's just the shadow of her hair making the right eye look kinda weird.
I've got no source but if you search for Fleshlight Creampie (Elle Rain aka Eeziss) you can find similar fleshlight creampie videos of her.
